There is no formal application process for entering the program.  It is open to current and prospective students.  The target audience is primarily undergraduate students in the IT and ICT program in SI (although any FSU undergraduate student may take the courses and apply for the certificate).  A student would start by taking the first class LIS 4785 (Intro to Health Informatics).  Internships are coordinated with professors.  Once the students have completed the certificate requirements, they will complete an internal school form that will provide the necessary information for issuing the certificate (name, courses taken, grades).  Though an application is not required, students interested in this program are encouraged to register their intent with the Certificate Program Coordinator.  Upon completion of the certificate requirements, students will email Assoc.
